"Open Test" option doesn't work.

In Test Explorer, there is an option to right-click on a test and "Open Test" (keyboard shortcut, F12). This feature doesn't do anything with this test adapter.

This is still broken because #3 was never implemented. Catch's --list-tests doesn't provide source/line info.
If memory serves, the XML reporter does provide the source location, but Visual Studio ignores attempts to set source/line that after test discovery.
