fix(github): keep ask citations public-safe

Motivation

• Prevent the public @gittensory ask path from publishing internal recommendation evidence (for example repo_decision summaries containing "ranked ... at priority ") into GitHub comments.
• Ensure ask citations surface only public-safe metadata about contributing sources rather than raw internal recommendationEvidence summaries.

Description

• Exclude private action-evidence sources from the ask citation path by skipping repo_decision in askSourcesFromActionEvidence via PRIVATE_ASK_ACTION_EVIDENCE_SOURCES and an early return null for those names.
• Stop forwarding source.summary verbatim into public citations by replacing the source detail with a fixed public-safe description via publicActionEvidenceSourceDetail(name).
• Add a unit test regression does not publish private repo decision ranking evidence in ask citations in test/unit/github-commands.test.ts verifying repo_decision origins are omitted and other public-safe sources remain cited.

Testing

• Ran the unit suite for the modified file with npm test -- test/unit/github-commands.test.ts and all tests in that file passed (16 passed).
• Ran the TypeScript typecheck with npm run typecheck (tsc --noEmit) which completed successfully.
• Verified no formatting/check errors with a local diff/check run which reported no issues.
