Implemented autonomous refactoring to replace ambiguous phrases with explicit hard constraints across all documentation files as part of the Anti-Hallucination Audit. Vague adjectives were quantified and weak modal verbs were replaced with strict directives (e.g. MUST, MANDATORY). All context-aware rules were respected (ignoring risk sections, protecting code blocks, adhering to TypeScript specifics), and new constraints were wrapped in GitHub Alert blocks. Updated the audit-log.md with the exact replacement count.
