Germany: ftChargeItemCase 0x…B1–B7 Ein-/Auszahlung with VAT

Part of fiskaltrust/market-de#294 (adjustment in docu).

Adds the new charge item case family 0x44450000000000B1–B7 to the German ftChargeItemCase reference table:

• VAT mapping like the other families (B1 19 %, B2 7 %, B3 10.7 %, B4 5.5 %, B5 not taxable, B6 zero, B7 unknown → UST_SCHLUESSEL 1–7 / > 1000).
• GV_TYP Einzahlung / Auszahlung with footnote: when to use them instead of the not taxable 0x…95 / 0x…96, direction by sign (negative = Auszahlung, positive = Einzahlung, reversed on void receipts) and an example of the signed process data.
