Fix the invalid streaming exception blocks.

Both helpers add an extra ) after the response_data expression. They also place a second except after the else suite. Python requires all except clauses to appear before else and finally, so this module cannot be imported. Remove the extra parenthesis and restructure the response-handling exception path as a nested try or as one outer try/except/finally block. (raw.githubusercontent.com)

Fix the response-handling blocks in both streaming helpers so the module parses:
remove the extra closing parenthesis after the response_data expression and
place every except clause before else or finally, using a nested try where
needed. Preserve the existing response and exception handling behavior.

Record request exceptions only once.

The existing handlers already call set_status() and record_exception(). These added lines call both methods a second time in _wrap and _awrap, which creates duplicate exception events for one failed request. Keep set_attribute(ERROR_TYPE, ...), but remove the repeated record_exception() and set_status() calls. (raw.githubusercontent.com)

In `_wrap` and `_awrap`, keep `span.set_attribute(ERROR_TYPE,
e.__class__.__name__)` but remove the duplicate `span.record_exception(e)` and
