fix(openai): record duration and token metrics for the Responses API - #4697
The Responses API wrappers (`responses_get_or_create_wrapper` and `async_responses_get_or_create_wrapper`) only received the tracer. The instrumentor never gave them the token-usage or duration histograms, and neither they nor `ResponseStream` recorded any metric. Responses calls produced spans but no `gen_ai.client.operation.duration` or `gen_ai.client.token.usage` points, on success or on failure.
Change
The Responses create/retrieve/parse wrappers, sync and async, now receive the token and duration histograms. Round 2:
- Duration is recorded only when a span is emitted, from the span's start time. Background polls and repeat retrieves add no points.
- Error points now carry operation, provider, server.address, stream, request model and `error.type`.
- A failure while processing a completed stream now records an error duration point.
Tests go through the real wrappers using `httpx.MockTransport`, sync and async: create (stream and non-stream), parse, 500 errors, a mid-stream `ReadError`, early close, and background create/retrieve/repeat. The new tests, `test_responses.py`, `test_exceptions.py` and ruff pass.
Behaviour changes: with metrics enabled, each emitted Responses span adds one duration point plus input and output token points. Failed calls and stream errors add one duration point with `error.type`. A stream closed early adds one duration point without tokens.
Not addressed: `responses.cancel` and `.with_raw_response.create(stream=True)` record no metrics, because cancel only closes an existing span and raw streams are untraced.

Package suites: no new failures against the offline baseline.
Ruff: clean.
Review
The Responses create/retrieve/parse wrappers, sync and async, now record gen_ai.client.operation.duration and gen_ai.client.token.usage once per emitted span. Call failures, stream errors and failures after stream completion record an error.type duration point. Early stream close records a duration point. Attributes match chat's. The tests use the real wrappers through httpx.MockTransport for sync/async, stream, parse, error and background cases. Remaining gaps are minor: test style, the "chat" literal, and no exception counter.

OpenAI Responses API telemetry now includes token usage and request duration for synchronous and asynchronous requests, including streamed responses. Duration is recorded when requests fail or streams end, while token usage is reported for completed responses. Metrics distinguish streaming from non-streaming activity and include response model and server details, giving users more visibility into usage and latency across response workflows.
The OpenAI Responses API instrumentation now records duration and token-usage metrics for synchronous and asynchronous calls. It also records duration metrics for request failures and stream lifecycle outcomes.
A telemetry wrapper forwards the tracer and optional metric instruments. Sync and async Responses registrations now pass the token and duration histograms.
Sync and async Responses calls record duration and token usage for completed responses, and duration for request failures. Tests cover success, server errors, and background response retrieval.
ResponseStream records duration and token usage on completion, duration on stream errors, and duration during cleanup. Tests cover broken streams and early closure.
Failed generations can appear successful in duration metrics, and some completed streams lose token metrics. Correct these gaps before merging.

Explanation
The title "fix(openai): record duration and token metrics for the Responses API" matches the main change. The diff adds duration and token-usage metrics to the sync and async Responses create, retrieve, and parse wrappers, and the linked issue #4585 asks for the same. The title does not name the streaming-duration or error-duration details, which is acceptable for a title.
Full details: Linked Issues check
Explanation
Issue #4585 requires duration and available token-usage metrics for synchronous and asynchronous Responses calls. The reviewed changes pass both histograms to create, retrieve, and parse wrappers. They record duration and input/output tokens for completed responses, including streamed responses. They record error duration points with error.type for call and stream failures. They record duration without tokens for early stream close. The tests cover sync and async success, streaming, parse, server errors, stream errors, early close, and background retrieval behavior.
Full details: Out of Scope Changes check
Explanation
The changed production files update Responses telemetry wiring and metric recording. The added tests exercise the linked behavior through mocked HTTP clients. No unrelated production behavior or unrelated tests are shown in the reviewed pull-request evidence. The documented exclusions for responses.cancel and .with_raw_response.create(stream=True) remain within the stated scope.

Mark failed Responses as errors in duration metrics.
A Responses call can return a terminal response with status="failed" instead of raising an exception. The sync and async terminal paths use _responses_metric_attributes for that response, so their duration points omit error.type. Stream completion uses the same helper. Check the terminal status and use the response error code, when available, before recording duration. Otherwise, failed generations appear as successful operations in metric queries. Record token usage when a completed stream closes early.
If a caller receives response.completed and then calls close() or aclose() before the iterator is exhausted, _process_chunk stores the completed response. Token usage is read only in _process_complete_response, which this path does not call. _ensure_cleanup then records only the duration point. The completed response therefore produces no gen_ai.client.token.usage points.
Call _process_complete_response() before _ensure_cleanup() when a terminal response is stored. Do not call it from inside _ensure_cleanup(), because _cleanup_lock is not reentrant. The method checks _cleanup_completed under the lock, so the later _ensure_cleanup() call returns early. A stream that closes before any response.completed event keeps the duration-only behavior.

Fixes #4585
Root cause
